The West Midlands renal network is launching its strategic framework for managing CKD in primary care on World Kidney Day. The framework aims to:
- raise awareness of the opportunity to identify and manage patients with Chronic Kidney Disease within primary care, where appropriate.
- give guidance on the process which will enable appropriate and timely referral of patients with CKD to secondary care for management by specialist teams delivering specialist care.
- offer a strategy for managing the renal QOF requirements and a mechanism to achieve key components of the NSF for Renal Services, Parts 1 & 2
- put forward recommendations to key stakeholders.
